Your access to and use of the Service is conditioned on your acceptance of and compliance with these Terms. These Terms apply to all visitors, users, and others who access or use the Service.

1. Consultation Agreement

​

By booking a telemedicine consultation with Dr. Louis Mandris, M.D. through 329 MJRX, you agree to the following:

​

• You are seeking consultation regarding medical cannabis certification in the State of Hawaii

• You understand that Dr. Mandris will evaluate your medical history and determine if you meet the qualifying conditions for a Hawaii 329 Medical Cannabis Card

• You consent to a telemedicine consultation conducted via video conference

• You will provide accurate and complete medical information

• You understand that certification is not guaranteed and is subject to Dr. Mandris's professional medical judgment

 

2. Payment Terms

The following payment terms apply to all consultations:

• New Patient Consultation: $97 USD

• 1-Year Renewal Consultation: $80 USD

• 2-Year Renewal Consultation: $120 USD

• Payment is required at the time of booking

• All fees are non-refundable once the consultation has been completed

• State registration fees ($38.50) are separate and paid directly to the Hawaii Department of Health

 

3. Cancellation & Refund Policy

 

We understand that plans can change. Our cancellation policy is as follows:

• Cancellations made at least 24 hours before your scheduled appointment may be rescheduled at no additional cost

• Cancellations made less than 24 hours before your appointment will not be refunded

• No-shows will not be refunded

• If we are unable to complete your consultation due to technical issues on our end, we will reschedule at no additional cost

 

4. Scope of Telemedicine Services

 

329 MJRX provides telemedicine physician consultation services only. Specifically:

• We are not affiliated with the Hawaii Department of Health

• We do not issue 329 Cards — only the Hawaii DOH can issue official cards

• We are not affiliated with any cannabis dispensary

• We do not provide medical advice beyond the scope of 329 Card certification

• We do not provide emergency medical care

• We do not prescribe controlled substances beyond what's required for 329 Card certification

 

5. Patient Responsibilities

 

As a patient, you agree to:

• Provide accurate and complete health information

• Complete the state registration process at MedMJ.ehawaii.gov

• Pay all required state fees directly to the Hawaii DOH

• Carry your 329 Card and photo ID when possessing or purchasing cannabis

• Comply with all Hawaii state laws regarding medical cannabis

• Notify us of any changes to your medical condition or medications

 

6. Limitation of Liability

329 MJRX and Dr. Louis Mandris, M.D. shall not be liable for:

​

• Any decisions made by the Hawaii Department of Health regarding your application

• Any legal consequences resulting from your use or possession of medical cannabis

• Any damages arising from the use or misuse of your 329 Card

• Any disputes with cannabis dispensaries

• Any indirect, incidental, or consequential damages

These Terms shall be governed and construed in accordance with the laws of the State of Hawaii, without regard to its conflict of law provisions.
